Penny, who also suffers from mental problems, adopted Arthur Fleck and took him to be cared for in a hostile and vulnerable environment, where her boyfriend even ties the child to a heater for a long period of time and causes psychological and physical trauma to his child head.

Arthur Fleck suffers from a disorder called Involuntary Emotional Expression Disorder, whose striking feature is uncontrollable laughter, but also exhibits symptoms of depression, eating disorders, hallucinations, and schizophrenia. He is neglected, ignored and attacked for his condition as everyone looks away from him on the streets - an apathy that is painfully portrayed in the movie.
