Arthur Fleck suffers from a disorder called Involuntary Emotional Expression Disorder, whose striking feature is uncontrollable laughter, but also exhibits symptoms of depression, eating disorders, hallucinations, and schizophrenia. He is neglected, ignored and attacked for his condition as everyone looks away from him on the streets - an apathy that is painfully portrayed in the movie.
