Penguin Books held a competition to design a cover for Ken Kesey's One Flew Over the Cuckoo's Nest. I wanted to keep the colour minimal using just red to highlight certain objects. Here we see the imagery of a cuckoo in a bed of screws, which represents the human psyche, with the cuckoo picking out a screw giving us the idiom of a "screw loose".
